Fill out your Mother's Name, Your Name, Phone Number, Email Address (Required) provided that the fieldhouse front desk, and tell us why your Mother is the Ultimate Soccer Mom and guess the number of soccer balls in the Honda Odyssey in front of the fieldhouse! Your mom could win a chance at a NEW 2005 Honda Odyssey or a $500 Gift Certificate to the Millcreek Mall!

The contest closes at midnight, April 15th, 2005 and the winner will be contacted within 3-7 business days of the contest closing.

Contest Rules:
1. The contest is open to the general public as well as anyone currently a member of or plays at Family First Sports Park. Employees of Family First Sports Park, its contest sponsor, advertising and promotional agencies and their respective affiliates and associates and such employees' immediate family members and persons with whom such employees are domiciled are excluded from this contest.

2. The prize is not redeemable in cash and must be accepted as awarded.

3. Decisions of the contest judges are final - no substitutions will be available.

4. By claiming the prize, the winner authorizes the use, without additional compensation of his or her name and/or likeness and/or voice/photograph and municipality of residence for promotion and/or advertising purposes in any manner and in any medium (including without limitation, radio broadcasts, newspapers and other publications and in television or film releases, slides, videotape, distribution over the internet and picture date storage) which Family First Sports Park may deem appropriate.

5. In accepting the prize, the winner, and any guest(s), acknowledges that Family First Sports Park may not be held liable for any loss, damages or injury associated with accepting or using this prize(s).

6. The person(s) whose name(s) is decided as being the winner(s) of the specific prize will be required to draw one envelope from 100. If you choose the envelope which contains the Honda Odyssey Picture—you will awarded the Odyssey. If you choose an envelope which contains a MillCreek Mall Logo, you will receive the $500 Gift Certificate to the Millcreek Mall.

7. Winners and any guests at the age of majority must sign a release form as prepared and accepted by Family First Sports Park acknowledging their acceptance and understanding of the complete contest rules.

8. Family First Sports Park retains the rights, in its absolute and sole discretion, to make substitutions of equivalent kind or approximate value in the event of the unavailability of any prize or component of the prize for any reason whatsoever.

9. This contest is subject to all federal, provincial and municipal laws.

10. Family First Sports Park reserves the right to withdraw or terminate this contest at any time without prior notice.
